Connect someone with Haven.

For physicians, social workers, case managers, community staff, and families.

What happens after you connect someone

  1. We contact the person you told us about, with their consent.

  2. We have an unhurried first conversation and review their coverage.

  3. We match them with an appropriate licensed clinician.

  4. We confirm back to you that we’ve connected, and, with the person’s permission, keep you informed as care proceeds.

What to include

Their name and how to reach them. Whether they know you’re connecting them with us (we’ll always ask their permission before beginning care). Your name, role, and contact. Anything you want us to know.

For clinicians

Haven treats depression, anxiety, grief and bereavement, adjustment difficulties, and related conditions in older adults through individual and group psychotherapy and behavioral health evaluations. We are not an emergency or crisis service and do not provide inpatient care. Positive depression or anxiety screens are welcome. You do not need a diagnosis to connect someone.

If someone is in immediate danger, call 911 or go to the nearest emergency room. If someone is in crisis, call or text 988, any hour, any day.
