The mood interview at start of care comes back positive more often than anyone says out loud. The patient is recovering from a hip, or a hospitalization, or a spouse’s death, and the depression or anxiety is real, treatable, and not what your agency is there to treat. Until now the options were a phone number that rings out or a clinic the patient can’t get to.
Why Haven fits home health
Sessions happen by phone, including a regular landline, so a homebound patient can start without a device, a ride, or a caregiver’s help. Psychotherapy from Haven’s clinicians is billed by Haven to Medicare Part B, separate from your agency’s episode. The discharge mood interview is a second chance to connect someone.
Does the patient need a physician order?
Not for psychotherapy under Medicare. If your agency’s process calls for physician notification, we’ll support it.
Will this interfere with our plan of care or our medical social work visits?
No. Haven provides psychotherapy, not medical social services, and we’ll coordinate with your team.
What do you need from us?
The patient’s name and phone, whether they’ve agreed to a call, your contact, and the PHQ score if you have it.
What happens after you connect someone

You send their details: secure form, phone, or fax.

We call the person, with their consent, and have an unhurried first conversation. A regular phone works fine.

We review their coverage and match them with a licensed clinician who has experience working with older adults.

We confirm back to you that we’ve connected, and with the person’s permission, keep you informed as care proceeds.
What we send back
Confirmation that we reached the person. With their consent, a brief intake summary in plain language and periodic updates at a cadence you ask for.
