For hospital discharge planners and case managers
Follow-through after discharge, by phone if that’s what works.
For discharge planners, social workers, and case managers in hospitals and rehabilitation facilities.
What you’re seeing
The patient going home Thursday is frightened, or grieving, or flat in a way the chart calls “adjustment.” The follow-up appointment you’d like to make doesn’t exist, or it’s in six weeks, or the patient will never get there.
How Haven fits
We can begin by phone from home. Ongoing psychotherapy with a clinician who works with older adults every day. Confirmation back to you, and with consent, updates. We make no claims about readmissions. We’ll show you our own numbers when we have them.
Can we connect someone before discharge?
Yes. Tell us the discharge date and the best number to reach the patient afterward.
What happens after you connect someone
-
You send their details: secure form, phone, or fax.
-
We call the person, with their consent, and have an unhurried first conversation. A regular phone works fine.
-
We review their coverage and match them with a licensed clinician who has experience working with older adults.
-
We confirm back to you that we’ve connected, and with the person’s permission, keep you informed as care proceeds.
What we send back
Confirmation that we reached the person. With their consent, a brief intake summary in plain language and periodic updates at a cadence you ask for.
How to connect someone with Haven
