Do residents need to leave the building?
No. Groups meet in your community’s own spaces, and individual sessions can happen by phone from a resident’s apartment.
What happens after you connect someone

You send their details: secure form, phone, or fax.

We call the person, with their consent, and have an unhurried first conversation. A regular phone works fine.

We review their coverage and match them with a licensed clinician who has experience working with older adults.

We confirm back to you that we’ve connected, and with the person’s permission, keep you informed as care proceeds.
What we send back
Confirmation that we reached the person. With their consent, a brief intake summary in plain language and periodic updates at a cadence you ask for.
